APPLICATIONS ARE NOW CLOSED
Application Deadline: April 30, 2008
Winners will be notified after July 15, 2008
Awards
NEW for 2008: Awards of up to $5,000 each will be provided to support pro bono projects and nongovernmental organizations where ACCP members volunteer their medical expertise in locations outside the United States and Canada for the 2008 award year. As in the past, all international projects/services require documentation, via a certificate or official letter, stating that the project is affiliated with a nongovernmental organization. Applications missing this important documentation will not be considered for an award. There are other important requirements listed below.
Award winners will also receive an engraved crystal award. All Humanitarian Recognition Awards applicants will be recognized in an awards booklet distributed at CHEST 2008 and on The CHEST Foundation Web site. The purpose of the Humanitarian Recognition Awards Program is to recognize ACCP members’ commitment to patient care through volunteer service. The Humanitarian Recognition Awards is a program of The CHEST Foundation, the philanthropic arm of the American College of Chest Physicians (ACCP).
Requirements
- Applicant must be a current member of the American College of Chest Physicians and must have been a member since April 2008. Applicants cannot apply for ACCP membership and an award concurrently.
- Applicants must be currently providing a voluntary service to a non-profit or non-governmental organization service project outside of the United States and/or Canada. These awards recognize volunteer service in existing projects. Award funds will not be granted to start-up projects.
- The service project’s primary intent must be to provide community service and not to conduct research (more information on research awards programs).
- All applications and supporting materials must be submitted in English.
- Continuing for 2008: Past winners of the Humanitarian Awards Program or the former Governors Community Service Awards are eligible to apply for the 2008 Humanitarian Recognition Awards. ACCP members who have previously received two Humanitarian awards cannot apply for additional funds in 2008.
- Past applicants to the Humanitarian Awards Program or the former Governors Community Service Awards program, who did not receive an award, may apply for the Humanitarian Recognition Awards by submitting their application in full by the appropriate deadline.
- Applicants may apply for the Humanitarian Recognition Award or the Humanitarian Project Development Grant, but not for both in the same year.
- It is highly recommended that the applicant include a letter of support from his/her Governor or International Regent as part of the supporting materials submitted with his/her application and a letter from the chief administrator (CEO, Executive Director) of the organization.
- A special Recognition Award will be granted on behalf of The CHEST Foundation’s Ambassadors Group.
Special Requirements for Award Recipients
- Award recipients will receive an award check of $4,000, issued to the nongovernmental organization/institution, after September 1, 2008. The remaining $1,000 will be issued upon receipt of the required Interim Report, which is due before February 1, 2009.
- Award recipients should try to attend the Making A Difference Awards Dinner to receive their personalized crystal award.
- Award recipients are also required to submit a final report of accomplishments, including budget accountability, which is due to The CHEST Foundation before September 30, 2009.
- An additional report, at least 3 years but no longer than 5 years from the date of receiving the award, is required to measure the long-term benefits of The CHEST Foundation’s funding of the volunteer project or service.
Application Process
Award recipients will be recognized during CHEST 2008, the ACCP Annual Scientific Assembly, at the 2008 CHEST Foundation Making A Difference Awards Dinner on Saturday evening, October 25, 2008.
