Tribute giving is a thoughtful way to honor someone on a special occasion or give a gift in memory of someone. If you are interested in donating to The CHEST Foundation in this way, you may do so online, by check, or by phone. If you are mailing your donation, please download the Honor & Memorial Tribute Donation Form and mail it in with your check(s). Your gift(s) are processed in the following manner.

• Your tax-deductible donation acknowledgement letter is prepared the same way, whether the gift is in memory of someone or in honor of someone. The letter states the donation amount along with the fund description, and it indicates the tribute information and the individuals who will be notified of your tribute gift.


• The notification letters vary slightly in their message depending on whether the tribute is in honor or memory of someone. In the case of a memorial gift, the family is sent a condolence letter that lists the donor's name and address (if multiple gifts are processed, a list is sent). When the gift is in honor of someone, the letter states why the gift is given, and it is usually addressed to the individual being named in the tribute, unless the donor indicates otherwise.


• At the donor's request, we will include a brief message within the body of the letter.


• All notification letters list the donor's name and address, so personal notes can be sent without further delay.


• At the end of each fiscal year, The Foundation includes an Honor and Memorial listing in our annual report.


• At year-end, The Foundation sends a copy of the annual report with a letter to all tribute donors and individuals or family members associated with the honor or memorial tribute.

• Process your gift online at The Foundation's secure donation site. All funds and endowment donations can be processed as a tribute gift.
