CCFAP Model Sites

To expand the study of the CCFAP model in other types of hospital critical care environments, The CHEST Foundation developed a Request for Proposal (RFP) process that extended an opportunity to replicate the range of services, materials, and tools for family assistance through the CCFAP model. Several CCFAP model sites were funded through this process, each representing a different model of care.
