The CHEST Foundation is pleased to introduce the Critical Care Family Assistance Program (CCFAP) Web site. Learn how hospitals around the country have designed and implemented this unique family-focused program in their critical care environments.
The site features overviews of program components, sample tools from the CCFAP Replication Toolkit, as well as photos and vignettes from the model sites.
The CCFAP model was created to respond to the unmet needs of families of critically ill patients in hospital ICUs through the provision of educational and family support resources.

Designing the development of an action plan includes both the content issues (what the plan should include) and process issues (how to organize the team) to achieve the plan.

Implementing the CCFAP provides strategies for identifying those organizational factors of personnel and resources that will ensure the success of the CCFAP through periods when obstacles will first be encountered and will need to be overcome.

Evaluating the CCFAP provides critical components for demonstrating the success of the program and the areas where it may be falling short.

Sharing the CCFAP includes ways to share program successes and challenges and lessons learned.
